What is your full name (name + surname)?
Interviewer instructions
You will administer this questionnaire to a group of several knowledgeable residents of the community, such as the village headman or his spouse, headmaster or teachers in the local school, agricultural extension officer, religious leaders, local merchants, health workers, long-term knowledgeable residents, or NGO workers that have been in the village for a long time. Choose informants who have lived in the community for a number of years. Make sure there is at least one representative from each village in the community. A minimum of 5 and a maximum of 10 respondents should make up the group that completes this questionnaire with you. The group should be as diverse as possible with respect to sex, age, religion and ethnicity, so that it is representative of the population in the community.

Note that in the questions in this questionnaire, the term COMMUNITY means the VILLAGE(S) OR PERI-URBAN LOCATION(S) IN WHICH THIS QUESTIONNAIRE IS BEING ADMINISTERED.
