<p>152. Column 49: Deaths to household members during the last 12 months.
<br />Make sure that the deceased was a member of the household at the time of his/her death, and make sure that the deceased died in April 1995 or after. List the name(s) of the deceased, sex and age at death. If the deceased is a woman aged 12-49 years, ask if she died while pregnant, giving birth or one month after giving birth.
