Total up the number of doses the mother reports (add doses in Qs. 3 and 5) and enter the total in the box next to Q. 7 (total doses received in lifetime). If she answered "doesn't know" to Qs. 3 and 5, enter 99 ("doesn't know") for her lifetime total.
