Interviewer instructions

Item 18: Survey code: Whether the originally selected sample household has been surveyed or a substituted household has been surveyed will be indicated against this item by recording code '1', if the sample household is the one originally selected, and code '2' if it is a substituted household. If neither the originally selected household nor the substituted household could be surveyed, i.e., if the sample household is a casualty, code '3' will be recorded. In such cases, only the Blocks 0, 1, 2, and the last two (remarks and comments) blocks will be filled in and on the top of the front page of the schedule the word 'CASUALTY' will be written and underlined.
