Interviewer instructions

This is a close-ended question with answers coded by numbers for each respective type of disease. For this question, the interviewer has right to read names of disease types listed in the answer part. In order to record questionnaire correctly, interviewer should ask into 3 independent questions, each question for one type of disease. For example, “Have you heard of gonorrhea?” Wait for an answer from the respondent, after that circle an applicable code. Then you continue to ask for two next diseases (Syphilis and Hepatitis B) so that enough 3 types of printed in answer part diseases are circled.
