If the respondent answers that he/she does not have a birth certificate because he/she hasn't picked it up from the civil registry office yet, or has lost it, circle number 1, "yes."<br /><span class="em">Birth Certificate</span>. This is the document that proves a person's identification by means of his/her registration with the civil registry office.<br /><span class="em">Civil Registry</span>. This is the public office in charge of recording births, marriages, deaths and other acts which alter a person's civil status. It issues certificates that prove a person's identity and maintains citizens' civil records.<br /></svar>
